Specialist Support Coordination

Specialist Support Coordination is a higher tier of support designed for NDIS participants:

• Facing complex barriers to accessing or maintaining services 
• Involved with multiple systems (e.g. justice, child protection, housing, mental health) 
• Experiencing crisis situations, instability, or high-risk factors
   

This service focuses on helping you resolve barriers, improve your support network, and maintain long-term, sustainable outcomes.

Our Specialist Support Coordinators provide:

✅ Expert coordination of complex services and providers
✅ Help navigating multiple systems (e.g. housing, hospitals, justice)
✅ Support during times of crisis or instability
✅ Building and coordinating a multidisciplinary team
✅ Support with NDIS plan implementation and reviews
✅ Creative problem-solving for complex situations
✅ Strong advocacy, especially when systems don’t work well together
✅ Focus on long-term stability and independence

We act as your lead coordinator, helping everyone work together to support your life—not just your plan.

Specialist Support Coordination is suited to NDIS participants who:

• Have very high or complex support needs 
• Are at risk of service breakdown or disengagement 
• Require intensive coordination between health, justice, housing, and disability systems
